Chapter 1 - The Doctor’s Verdict and the Shadow of Treason

The sterile fluorescent lights of Chicago Memorial Hospital hummed with an agonizing, clinical indifference. Dante Moretti stood in the center of Trauma Bay 4, an imposing monolith dressed in a tailored charcoal Brioni suit that still smelled of rain, gunpowder, and vintage scotch. His chest heaved against his crisp silk shirt, his jaw clenched so tightly the muscles threatened to snap. On the examination table lay Isabella—his wife, the woman who had carried his name in silence while he waged blood-soaked wars across the city’s underground.
She was eight months pregnant, her petite frame utterly dwarfed by the heavy hospital linens. Her skin was the color of fresh ash, her breathing shallow and ragged. But it was not just her pallor that made Dante’s blood run to ice. It was her right wrist.
Dante reached out, his massive, scarred hand trembling with a terrifying, contained violence. He wrapped his fingers gently beneath her arm, lifting her limp hand into the light. Four distinct, purple-black contusions ringed her delicate bone like a barbaric iron shackle—the indisputable imprint of malicious, crushing force. Next to her on the stainless-steel tray lay the crumpled, blood-spotted divorce papers Vanessa had brandished in Isabella’s face only an hour prior.
"Who did this?" Dante’s voice was barely louder than a whisper, yet it carried the dead weight of an execution order. The air in the room dropped ten degrees. Two armed enforcers standing guard at the door instinctively shifted their footing, their hands hovering near their concealed holsters. They knew that quiet register. It was the tone Dante used before he burned city blocks to the ground.
Dr. Kenneth Reynolds, the chief of maternal-fetal medicine, stepped into the bay. He held an acrylic tablet, his older face drawn with grave exhaustion. He glanced from Dante’s murderous eyes down to Isabella’s swollen belly, where a fetal heart monitor beeped with an erratic, distressingly rapid rhythm.
"Mr. Moretti," Dr. Reynolds began, his voice tight. "You need to hear this before you make any decisions. And you need to brace yourself."
Dante did not blink. His eyes remained locked on the bruises on Isabella’s wrist. "Speak, Kenneth. Before I lose what little patience I have left."
"First... your wife was not carrying a single child," Dr. Reynolds revealed, his voice solemn. "The ultrasound performed in triage just confirmed twin gestations. A boy and a girl."
A sharp, violent shock cracked through Dante’s chest. *Twins.* Two heirs. Two heartbeats he had never known existed because he had been too consumed by syndicate wars, too distant, too blinded by the deceptive peace Vanessa had painted around him. But before the warmth of fatherhood could take root in his dark soul, Dr. Reynolds shattered it completely.
"The fall down the staircase caused a partial placental abruption in twin A," Dr. Reynolds continued, pulling up the radiographic scans. "That alone would have put them in severe peril. But that is not what caused her collapse, Dante."
Dante straightened to his full six-foot-three height. "What are you saying?"
"We ran an urgent toxicology screen because her blood pressure was crashing inexplicably," Dr. Reynolds said, stepping closer and lowering his voice so only Dante could hear. "We found lethal concentrations of thallium sulfate in her bloodstream. It is an odorless, tasteless cellular toxin. It has been administered to her in micro-doses over the course of at least four weeks. Whoever pushed her tonight didn't just cause an accident, Dante. Someone has been systematically poisoning your wife and your unborn children from inside your own home."
The room plunged into absolute, suffocating silence. Dante looked at Isabella’s pale, trembling lips. The memory of Vanessa standing haughtily in his penthouse, playing the doting consort while Isabella withered away in the master wing, exploded behind his eyelids like white phosphorus.
"The toxin compromised the placenta's vascular wall," the doctor added softly. "The physical blunt-force trauma from being shoved onto the floor triggered massive internal hemorrhaging. If we do not stabilize her within the hour, both babies will asphyxiate. And Isabella’s heart will give out."
Dante leaned over his unconscious wife. With an agonizing tenderness completely alien to his ruthless reputation, he brushed a damp strand of dark hair from her forehead. His thumb traced her bruised wrist.
"Hold the surgery until I give the command to seal the floor," Dante ordered, his voice terrifyingly calm, venom dripping from every syllable. "Nobody enters this wing without my personal biometric clearance. If a single nurse breathes wrong near her bed, shoot them."
He turned to his lieutenant, Marco, who stood white-faced near the door.

"Call the penthouse," Dante commanded, his eyes burning with an infernal fury. "Tell Vanessa to stay right where she is. Tell her... I’m coming home to sign the papers."
